At Freestone Medical Center, our top priority is to protect and secure the privacy of your health information. All information contained within your medical record is considered confidential and is treated with the highest regard for integrity, security and safety. 

While the medical record remains the property of Freestone Medical Center, the information contained therein belongs to you, the patient. Copies of your health records can be obtained upon written request/release from/by you.

To request copies of your medical records, please complete an authorization form (click here) and mail, fax or personally bring the form to FMC along with a copy of your driver’s license to the Health Information Management (Medical Records) Department at the address listed below. Please be sure to include your telephone number so we can contact you in case we have questions regarding your request.

Radiology images needed for a specialist visit may be obtained with the same release form below, but are provided directly from the Radiology Department.  We will coordinate and help you with obtaining images.

Additionally, any of your treating physicians can request your medical records be sent to their office.  Most physicians have a records release form for your use.
