Freestone Medical Center provides skilled nursing services (swingbed) for patients still recovering from an acute hospital episode or surgical intervention, but have been deemed to be ready for discharge from the “acute” phase of inpatient hospital care.
Freestone Medical Center offers skilled nursing care and rehabilitation for those patients that qualify. If you are not quite ready to recuperate at home or have been told by your physician that you need “continued rehab” then Freestone Medical’s skilled nursing care may be just what you are looking for. Our nursing staff does more than administer medication and bedside care. We are patient advocates who take pride in the care that we deliver to the residents of Fairfield and the surrounding community.
Swing Bed Services
We provide swing bed (or skilled nursing) care for patients who aren’t quite ready to go home and need a bit more care after their surgery or after a long inpatient hospital stay. If you are suffering from any of the following clinical categories and are unable to safely return home, our swing bed program may be an appropriate option for you:
- Major Joint Replacement or Spinal Surgery
- Non-Surgical Orthopedic/Musculoskeletal
- Orthopedic- Surgical Extremities NOT Major Joint
- Acute Infections/Weakness
- Medical Management
- Cancer
- Pulmonary
- Cardiovascular and Coagulations
- Acute Neurologic
- Non-Orthopedic Surgery
Our available therapy programs include:
- Physical Therapy
- Occupational Therapy
- Speech Therapy
We accept orthopedic referrals from other hospitals, who need additional long-term care or any other type of orthopedic surgery that qualifies for swing bed care. FMC also serves those in need of long term I.V. antibiotic administration in a skilled nursing setting.

Many patients are transferred to nursing homes from other hospitals because they may not be aware that we have swing bed care right here at Freestone Medical Center. Also, younger patients who need swing bed care after a major orthopedic procedure may be more comfortable in our facility in lieu of a nursing facility.
Yes. Unlike most nursing homes, we allow each patient one visitor per day if they’re admitted to our swing bed program.
Our skilled nursing team works directly with our incredible doctors the entire time that you are in the hospital. The interdisciplinary team of nurses, therapists and dieticians will deliver exceptional care to you and make sure that the doctor’s recommendations for care are followed until you go home.
No. Swing Bed (skilled nursing) requires a physician’s order. However, you may request that your physician refer you to Freestone Medical Center’s Swing Bed program.
